Harm reduction has been a cornerstone of the HIV and hepatitis C response
since the early days of the epidemics. Despite initial skepticism and
resistance, needle and syringe programs are now widely accepted as an
effective public health intervention. But new developments in drug use
patterns and the health issues facing people who use drugs have prompted us
to think more broadly about the concept of harm reduction.

Canada is in the midst of an overdose crisis, which has already taken the
lives of 9,000 people in less than three years

The culprit is a poisoned drug supply, which needle and syringe programs
cannot resolve. The solutions are more complicated, and require radical
changes to how we deliver harm reduction services and how we address drug
use as a society.

We have also seen how drug use relates to HIV and hepatitis C beyond
needles and syringes. Studies of gay and bisexual men have found
associations between sexualized drug use ("Party and Play" or "chemsex")
and HIV and hepatitis C transmission, even in the absence of injection
drugs.

These are just two examples that illustrate the urgent need to expand our
harm reduction approaches in order to meet the health needs of people who
use drugs. In this issue of the *CATIE Quarterly*, we spotlight key
resources to update our harm reduction knowledge for 2019 and improve our
programs for people who use drugs.

27% surge in admissions to treat these poisonings, and a 73% increase in
