In the history of HIV and hepatitis C, we have never been as close as we
are now to eliminating the epidemics from our country. Effective
antiretroviral treatment prevents HIV transmission and gives HIV-positive
Canadians life expectancies similar to their HIV-negative peers.
Direct-acting antiviral medications cure most cases of hepatitis C in a
matter of weeks.

With these tools at our disposal, eliminating HIV and hepatitis C
transmission is possible, and our government has signed on to global
targets to achieve this by 2030.

What stands in our way is the large number of Canadians who aren’t
accessing treatment because they don’t know they are affected. An estimated
14% of HIV-positive Canadians don’t know their HIV status, and a daunting
44% of Canadians with hepatitis C remain undiagnosed. A 2018 survey found
that 50% of Canadians have never been tested

for a sexually transmitted or bloodborne infection.

This needs to change, and it starts with expanding and piloting
different approaches to testing. In this issue of the *CATIE Quarterly*, we
spotlight ways that Canadian service providers are bringing the offer of
HIV and hepatitis C testing to the people.

We can’t expect to find the undiagnosed without changing our approaches to
testing. Rather than waiting for individuals to present themselves at a
testing clinic, novel approaches bring the offer of testing where people
are – in communities, in non-clinical settings, even in homes. Testing is
important as it is not only the gateway to care and treatment for those who
test positive, but also a gateway to prevention for those who test
negative. Read more.

Research tells us that most HIV transmissions originate from people who
think they are HIV-negative but have recently contracted the virus — the
undiagnosed. They may have never been tested or they may have been infected
since their last test. And there are a few reasons why most HIV
