Peer Navigation Services Coordinator
Reporting to: Director of Health Promotion
Start Date: March 1 (or as soon thereafter as possible)
Hours of Work: 35 hours per week
Benchmark: Program Coordinator 1
Grid Level: 10 (Wage Scale: $20.51-$22.23/hour)

Position Summary:
BCPWA is a consumer-led, member driven, registered charitable society run by
and for persons living with HIV and AIDS. BCPWA is Western Canada's largest
AIDS organization with a membership of more than 4,900 HIV-positive members.
The Peer Navigation Services Coordinator assesses member/participant needs
related to health, wellness, social and emotional well-being, and, in
conjunction with the member/participant, develops strategies to maintain or
improve health and wellness needs and to implement self-management
skill-sets. Working collaboratively with other healthcare providers, the
Peer Navigation Services Coordinator provides direct system navigation,
advocacy, education and related linkage to relevant specialized health and
wellness resources related to HIV. Networks with front line service
providers and builds collaborative inter-agency partnerships in order to
promote Peer Navigation Services and addresses issues related to people
newly diagnosed with HIV or seeking to develop HIV related self-management
skills.
Duties:
1. In consultation with the Director of Health Promotion, undertakes
the planning, development, delivery, monitoring and evaluation of Peer
Navigation Services.
2. Assesses member knowledge and self-management skills in the areas
of HIV disease in the areas of treatment, preventing disease progression,
self-care, emotional wellness, social support, disclosure and
harm-reduction;
3. Aids in the development of self-management plans based upon
member-identified needs, and identify tasks and optimal timelines;
4. Ensures on-going support to members by developing mutually agreed
engagement plan through contact via appointments, e-mail and telephone
calls;
5. Refers members to HIV related health and well being support
services and/or other community services;
6. Advocates on behalf of clients to address barriers to accessing
service;
7. Networks with service providers in order to build collaborative
inter-agency partnerships and referral protocols that promote the Peer
Navigation Services for people newly diagnosed with HIV or ready to reengage
in care, treatment and support;
8. Visits clinics or community agencies or accompanies nurses to
offsite locations in order to reduce time between point-of-diagnosis and
access to care, treatment and support;
9. Develops and maintains record keeping, case management systems, and
resources related to the program;
10. In consultation with the Director of Health Promotion, adjusts program
on an ongoing basis to address recommendations, deficiencies and
sustainability of the project revealed in member/participant and facilitator
feedback;
11. Trains and supports Peer Navigator Team;
12. Co-facilitates support groups in conjunction with clinic facilitator
in partner clinics where HIV+ people attend;
13. Attends external agency meetings related to people newly diagnosed
with HIV;
14. Plans and delivers workshops related to people newly diagnosed with
HIV for frontline workers;
15. In consultation with the Director of Health Promotion, develops
program monitoring and evaluation tools to measure outcomes;
16. Participates in professional development opportunities as required;
17. Attends all staff meetings as called by the Executive Director and/or
the Director of Health Promotion;
18. Provide training, leadership and supervision to related volunteers of
Society;
19. Participates ex officio on the Health Promotion Standing Committee;
20. Assists the Director of Health Promotion in the preparation of funding
initiatives to government and other funding bodies;
21. Maintains program statistics and prepares statistical summaries and
other reports as required and assigned;
22. Participates in the annual strategic planning of the Society and Peer
Navigation Services programming in collaboration with key volunteers and
staff;
23. Follows budget designated to program;
24. Performs other related duties as assigned.
QUALIFICATIONS:
Education, Training and Experience
. Bachelor's degree in a health related discipline such as Social
Work, Family Sciences or Psychology with competencies in assessment, case
management and counseling of people with chronic disease and three years
case management experience in an HIV community setting or an equivalent
combination of education, training and experience;
. A combination of education and work experience in workshop design
and group presentations;
. Must be HIV+ and comfortable with sharing your HIV status with
clients and public;
. Knowledge and understanding of principles of HIV self-management;
. Proven knowledge and understanding of best practices as it relates
to HIV care and associated co-morbidities;
. Proven in-depth knowledge of the determinates of health and the
psychosocial, physical and functional problems endemic to HIV;
. Skills in support group facilitation;
. Strengths in community-based program planning, development and
evaluation;
. Direct work experience with the gay, bisexual, transgendered,
injection drug using and/or refugee communities;
Skills and Abilities:
. Proven ability to foster and improve relationships between
professionals, programs and agencies;
. Excellent written and oral communication skills, computer skills
and experience with information technology;
. Demonstrated ability to work independently and cooperatively
within an interdisciplinary team;
. Superior interpersonal skills and demonstrated ability to interact
with healthcare professionals and consumers;
. Knowledge of issues related to diversity, and to the needs of
persons living with HIV/AIDS;
. Ability to plan, organize and prioritize;
. Physical ability to carry out the duties of the position;
. Proven ability to work with and supervises volunteers;
. Proven leadership, problem solving and organizational skills;

Community Sub-sector Collective Agreement. CUPE Local 3495.
