tedious repeat.)
Dear folk,
Okay. Things are starting to move very rapidly in very
unpleasant directions.
Attached is a copy of the January 11, 2011 “Designation Order”
which spurs and governs the further development of the PLIS-HIB (Provincial
Laboratory Information Solution - Health Information Bank – this is the
eHealth “repository” databank which will store all laboratory work results –
see article 2.2 – and make those results available as provided in the
designation order).
The Order provides that all lab work done in Northern and
Interior Health Authorities will now be deposited in the PLIS-HIB (where it
will join the lab work already being deposited from the Provincial Health
Services Authority, principally from the BCCDC).
The astonishing thing in this order is the scope of persons to whom it
grants “roll-based access” to the information dumped into the PLIS-HIB.
These include physicians, of course. But also included are such “roles”
as: midwife, physiotherapist, dietician, and a host of similar others *and
those studying to become such professionals*. (See schedule 4-1 on page
12.)
So far, under this Order, such role-based access is so widely granted only
in Vancouver Coastal (presumably including Providence) and Provincial
Health; oddly, both Northern and Interior Health are currently restricted to
“Clinical Validator” roles (schedule 4, page 10). But that will change.
Possibly quite soon.
Given the way this is now rolling out, it is imperative that systems be put
in place as quickly as possible to advise patients/clients of these
developments, and to assist them, should they wish it, in securing the only
protection available to them: disclosure directives on the PLIS-HIB, and
“lock downs” in the pertinent Health Authority personal health information
management systems.

P.S.: For those of you who haven’t yet seen it, attached, too, is a copy of
the Canadian Medical Association’s recently released “Principles for the
Protection of Patients’ Personal Health Information”, undated but copyright
2011. Note, in particular, section 13 on page 4, which starts as follows:
“13. Patients should be informed that the treating physician cannot control
access and guarantee confidentiality for an electronic health record (EHR)
system.”
P.P.S.: So ... when might we look forward to receipt of the newly revised
guidelines?

Dear Elizabeth and All,
After lengthy consideration in its Community Representation and
Engagement (CRE) Standing Committee one week ago today, and upon receipt of
that Committee’s recommendation, the BCPWA Board of Directors this afternoon
formally adopted the position statement on pre- and post-test counselling
and disclosure directives attached to this email.
On the basis of that position statement a series of amendments
are suggested to the draft Pre- and Post-Test Counselling Guidelines
previously circulated. These suggestions are made using “Track Changes” in
the originally circulated document, also attached.
BCPWA is most grateful for the opportunity to offer these
suggestions. We realize that they are very difficult. We do truly wish
there were a workable alternative; unfortunately, in the current climate of
careless disregard for individuals’ privacy rights codified in provincial
statute, we see no such alternative.

This document has a number of hoops still to jump through so I will continue
it on its path and will greatly appreciate your comments Ross.

Hello All,
Thank-you for offering to review the attached Pre and Post Test HIV
Guidelines.
The guidelines have undergone a preliminary review by the STI/HIV clinical
leadership team at BCCDC and that feedback has been incorporated into the
document that you will be reviewing titled Draft #2. There are some
comments and track changes that persist in your copies because these need
further consideration and follow-up on my end.
This document will be heading out to the provincial working groups the first
week of January and to the broader provincial CD working groups. I
recognize that the holidays are right around the corner, but please provide
your feedback by December 30 (please use track changes if possible - it
makes it much easier to see and incorporate feedback).
I will be sending the 3rd draft that incoporates your changes on Tuesday
January 4. I am hoping that it is a relatively easy read.
As you will see, the appendices remain works in progress and will be further
developed over the next few weeks; however, these will not change the
overall gist of the document.
In your review, I am particularly looking for:
- any obvious typos
- guideline construction that is confusing
- content that contradicts itself or other documents currently in use
- ease of the reader in discerning the differences and
commonalities between POC and standard lab testing pre and post discussions
- any missing or needed definitions
- bits that you think need to be added that are essential to the
discussion with clients and would not be covered in pre and post test
education sessions provided to health care providers
Additionally, the pregnancy component is purposefully absent because of the
current BC guidelines that exist related to perinatal testing.
